For your privacy, please do not email completed forms. My intake form is quite long. You will probably need close to an hour to complete it. When thoroughly filled out, it provides me with valuable information, and saves you time in session. Please fill out as much as possible and bring it with you to our first meeting.

We will also sign the consent and HIPAA forms at our first meeting. Feel free to print them and bring them with you or sign the ones I provide when we meet.

Clinical training and experience

Masters in Counseling Psychology. Varied/Integrated clinical training including: Psychodynamic, Narrative, and Relational approaches.

Hakomi: a combination of body-centered therapy, mindfulness, and experiential therapy that gently and powerfully deepens self-knoweldge and integration of different parts of ourselves.

EMDR (Eye Movement Desensitization & Reprocessing): a brief therapy technique for the relief of symptoms caused by traumatic life experiences.
